When Is Wisdom Tooth Removal Necessary?

Not every wisdom tooth needs to come out. If a tooth erupts fully, sits in proper alignment, and can be cleaned effectively, it may be safe to monitor. But this is the exception. Most wisdom teeth either fail to erupt completely or grow in at an angle that creates ongoing risk. There are several specific situations where removal is the right call:

Impaction. A wisdom tooth is impacted when it cannot fully break through the gumline. Partial impaction — where the tooth partially emerges — creates a flap of gum tissue that traps bacteria and is nearly impossible to clean, leading to infection (pericoronitis), decay, and swelling. Full bony impaction means the tooth is entirely enclosed in the jawbone, where it can silently damage adjacent roots or form cysts over time.

Root and nerve damage. Wisdom teeth roots are unpredictable. As they develop, they can grow toward the roots of the second molars, weakening or fracturing them. In the lower jaw, roots can also grow close to the inferior alveolar nerve, which requires careful evaluation before extraction.

Cysts and bone loss. An impacted tooth sits inside a follicular sac. If that sac fills with fluid and becomes a cyst, it can erode the surrounding jawbone and damage nearby teeth — sometimes without causing pain until significant harm has already been done. Regular X-rays are the only reliable way to catch this early.

Crowding and shifting. Pressure from erupting wisdom teeth can push adjacent teeth out of alignment, affecting your bite and undoing orthodontic work. Even patients who’ve had braces may notice shifting when wisdom teeth are left in place too long.

What to Expect During Chicago Wisdom Teeth Removal

Incision and exposure. For impacted teeth, we make a small incision in the gum tissue to access the tooth and remove any bone covering it. Fully erupted teeth typically require no incision.

Extraction. We remove the tooth, breaking it into smaller pieces if needed to minimize the size of the opening. Most extractions take 30 to 60 minutes total, depending on how many teeth are being removed and their position.

Closure. We close the incision with dissolvable stitches, place gauze over the extraction sites, and go over your aftercare instructions before you leave.

Recovery. Most patients feel back to normal within 7 to 10 days. Swelling and discomfort are usually most noticeable in the first 2 to 3 days and improve steadily after that. Plan to rest for the remainder of the day of your procedure, stick to soft foods for the first several days, and avoid straws, smoking, and vigorous rinsing during the first 24 hours to protect the healing clot. Full socket healing takes 3 to 4 weeks.
